Additional Employer Information

You will need to complete this section to provide information on the payroll schedule, open enrollment period and other information needed to install the case.

Payroll Schedule

The Start and End date will default based on the Effective Date set in Employer Demographics.  You will need to enter a First Day of Payroll between the start and end dates, and also select a Payroll Frequency (ex: Weekly, Bi-Weekly, etc.)

Open Enrollment Window

The Start Date should be before the Effective Date and not before the present date. The End Date cannot be greater than 30 days.

Additional Information

Answer the Yes / No questions in this section regarding Public Entity, ERISA, Domestic Partnership and TEFRA.  You will also be asked to include the Agent Contact Number for support.

Tip

The Agent Contact Number will display on the Employer/Employee Portal if you choose the self-enrollment option!

Information Required for Ancillary Products

Enter the number of hours per week an employee must work to be considered full time.

Eligibility Date

Select the appropriate eligibility / waiting period for enrollment in the plan.
